To complete the form:
• Please complete a separate form for each project completed.
• You must complete all text fields. An incomplete form will delay the process.
• Submit any supporting documentation, including registration forms, photos, etc.) to the Fort Bliss Army Volunteer Corps Office via email to tephanie.l.hopper.civ@mail.mil or wendy.m.foxx.civ@mail.mil, or in person at Army Community Service, Bldg. 2494 Ricker Road.
• Volunteer projects must also be registered in VMIS at www.myarmyonesource.com.

All project submission forms must be submitted no later than January 12, 2012.
